ARC Review Request Form (public)

Please download and fill out this form to submit your ARC requests. This form can also be used to request tree removals. Please attach details with pictures for all your ARC Requests.

For tree removal requests, provide the location of each tree, dba (diameter at breast height) and species. Trees greater than 6 inches require ARC approval and trees 18 inches and larger require a PTC permit. Please indicate which trees are dead and which are dying, if applicable.
